Starting a new job is an exciting time, but it can also feel a bit overwhelming with all the information to take in and forms to complete. That's why we've created this one-stop orientation webpage to make your onboarding process as smooth and organized as possible.

Here you will find all the documents, forms and resources you need to get off to a great start in your new role with the Town. From required paperwork like tax forms and direct deposit authorization, to information on employee benefits and Town policies, this page has you covered. If you can't find what you're looking for, just reach out to the Town Administrator's Office and we'll be happy to assist you.

Required Reading - Personnel Policies

Payroll Documents

  • I-9 Form
    The I-9 form is used to verify an employee's identity and eligibility to work in the United States. Employers are required to complete this form for all new hires, and employees must provide acceptable documentation to establish their identity and employment authorization.
  • W-4 Form
    The W-4 form, also known as the Employee's Withholding Certificate, is used by employees to indicate their federal income tax withholding preferences. This form determines the amount of federal income tax that an employer will withhold from an employee's paycheck.
  • M-4 Form
    The M-4 form is the Massachusetts equivalent of the federal W-4 form. It is used by employees working in Massachusetts to indicate their state income tax withholding preferences.
  • Worcester County Retirement
    This document relates to the retirement system for employees working in Worcester County, Massachusetts. It contains information about enrollment, contributions, and benefits associated with the county's retirement plan.
  • SSA Form
    The SSA form, or Social Security Administration form, is used to report an employee's Social Security number to the Social Security Administration. This form ensures that an employee's earnings are properly credited to their Social Security record.
  • Direct Deposit Authorization Form
    This form allows employees to authorize their employer to deposit their paycheck directly into their bank account. Providing direct deposit information is typically optional, but it can be a convenient way for employees to receive their pay without the need for a physical check.

HR Documents

  • CORI Acknowledgment Form
    The CORI (Criminal Offender Record Information) Acknowledgment Form is used to obtain an employee's consent for the employer to conduct a background check. By signing this form, the employee acknowledges that they understand the purpose of the background check and authorize the employer to obtain their criminal record information.
  • Confirmation of Receipt of Town Policies 
    This document confirms that the employee has received and read the various town policies related to HIPAA (Health Insurance Portability and Accountability Act), personnel matters, internet and computer use, vehicle use, and workplace violence. The employee acknowledges their understanding of these policies and agrees to abide by them.
  • Confirmation of Receipt of Equal Employment Opportunity, Discrimination, and Sexual Harassment Policy 
    This outlines Hubbardston's commitment to providing equal employment opportunities and maintaining a work environment free from discrimination and sexual harassment. It defines these terms, provides examples of prohibited conduct, and outlines the procedures for reporting and addressing such incidents.
  • Conflict of Interest Law Summary & Acknowledgment
    The Conflict of Interest Law Summary provides an overview of the state's laws regarding conflicts of interest for public employees. The acknowledgment form is signed by the employee to confirm that they have read and understood the summary and agree to comply with the law.
  • Conflict of Interest Law – Training
    This refers to the actual training course or module that employees must complete to fulfill their obligation under the state's Conflict of Interest Law. The training covers topics such as gifts, outside employment, and the use of public resources, among others. Please return your certificate at orientation.
